Case Manager (Experience with Senior Citizens)

JOB TITLE:              CASE MANAGER
LOCATION:              New Rochelle & Yonkers

REPORTS TO:         Service Coordinator

SUMMARY:
Under the supervision of the Service Coordinator, the case manager will coordinate services and provide supportive counseling to the seniors residing in two Westhab buildings, located in New Rochelle and Yonkers.

DUTIES & RESPONSIBILITIES:

  • Must be able to independently identify and assess senior citizens’ needs for services, develop a service care plan, monitor and provide follow-up.
  • Must have knowledge of community resources, how to make appropriate referrals and coordinate services.
  • Serve as an advocate/liaison in assisting seniors in obtaining social, medical, financial, psychological, educational and nutritional services.
  • Must have knowledge and skills in the use of computer software applications for drafting documents, data management and tracking
  • Must maintain complete records, reports and statistical data as required by departmental policy, federal and state laws
  • Provide supportive counseling and assist the seniors with public benefits applications and other entitlements programs and opportunities
  • Must have knowledge and familiarity with medical and mental health diagnoses, disabilities and treatment
  • Participate in case conference meetings /discharge planning as deemed appropriate 
  • Conduct home visits, as needed
  • Conduct or coordinate group workshops/recreational activities/programs on a variety of topics
  • Perform other job related duties as assigned by supervisor

EXPERIENCE, EDUCATION AND SPECIAL REQUIREMENTS:

Bachelor’s Degree preferred and at least 3 years of demonstrated case management experience.  Experience working with seniors, preferred. Must have strong written and verbal communication skills. Provide good customer service (both in person and by telephone). Fluency in Spanish is highly desirable.  Must have own car and a valid NYS drivers license required.
