JOB TITLE: CASE MANAGER
LOCATION: New Rochelle & Yonkers
REPORTS TO: Service Coordinator
Under the supervision of the Service Coordinator, the case manager will coordinate services and provide supportive counseling to the seniors residing in two Westhab buildings, located in New Rochelle and Yonkers.
DUTIES & RESPONSIBILITIES:
EXPERIENCE, EDUCATION AND SPECIAL REQUIREMENTS:
Bachelor’s Degree preferred and at least 3 years of demonstrated case management experience. Experience working with seniors, preferred. Must have strong written and verbal communication skills. Provide good customer service (both in person and by telephone). Fluency in Spanish is highly desirable. Must have own car and a valid NYS drivers license required.
