JOB TITLE: Senior Case Manager
DEPARTMENT: Rapid Re-housing
LOCATION: Various Sites
REPORTS TO: Retention Service Coordinator
SUMMARY: Supervises the retention program’s case managers. Provides case management and supportive services to residents and ensures that all clients enrolled in retention programs are assessed and referred for the appropriate services. Attends all necessary program meetings scheduled with collateral agencies.
DUTIES & RESPONSIBILITIES:
EXPERIENCE, EDUCATION AND SPECIAL REQUIREMENTS:
Bachelor’s Degree or equivalent work experience required. Strong computer skills. Excellent case assessment and management skills. Strong verbal and written communication skills. Demonstrate ability to plan, to organize, and evaluate scope of services. Bi-lingual (Spanish) a plus. Position requires a valid driver’s license and own vehicle.
