Senior Case Manager - Yonkers, NY

JOB TITLE:                            Senior Case Manager  

DEPARTMENT:                      Rapid Re-housing

REPORTS TO:                      Retention Service Coordinator

SUMMARY:   Supervises the retention program’s case managers. Provides case management and supportive services to residents and ensures that all clients enrolled in retention programs are assessed and referred for the appropriate services. Attends all necessary program meetings scheduled with collateral agencies.

DUTIES & RESPONSIBILITIES:

  • Supervise Retention Case Managers.
  • Responsible for an individual Family and Single retention program caseload.
  • Conducts regular supervision and annual formal performance appraisals for direct reports.
  • Plans and organizes a range of casework services to meet client needs.
  • Provides support to program staff in identifying client needs.
  • Responsible for developing service plans with residents. Must communicate with all collateral agencies to ensure compliance with ILP’s.
  • Responsible for conducting outreach to shelters, conducting intake interviews and processing move-ins of new residents, monitoring and maintaining full program occupancy.
  • Responsible for preparing reports and analyzing as required.
  • Develops and implements processes to meet compliance requirements.
  • Develops systems to guarantee the quality and efficiency of retention services.
  • Coordinates with necessary departments to ensure all apartments operate safely, comfortably and are in compliance with housing quality standards.
  • Oversees and performs required data entry.
  • Responsible for the submission of required documentation.
  • Attends required meetings.
  • Other duties as needed.

EXPERIENCE, EDUCATION AND SPECIAL REQUIREMENTS:

Bachelor’s Degree or equivalent work experience required. Strong computer skills. Excellent case assessment and management skills. Strong verbal and written communication skills.  Demonstrate ability to plan, to organize, and evaluate scope of services. Bi-lingual (Spanish) a plus. Position requires a valid driver’s license and own vehicle.
