Case Manager

JOB TITLE:                 Case Manager (HITE)

LOCATION:                 Westchester County, NY  

REPORTS TO:            Service Coordinator

 

SUMMARY OF DUTIES: Directly provides intense case management, counseling, life skills coaching, crisis intervention and programmatic tasks for formerly homeless individuals while assisting those with significant barriers to become motivated to seek and obtain assisted competitive employment. Promote independency and self-sufficiency through “hands on” approach to resolving barriers to employment accompanied by independent living.

DUTIES & RESPONSIBILITIES:

As it pertains to Housing:

  • Responsible for individual caseload of single and family adult participants
  • Oversees apartment finding and/or referrals for housing assistance
  • Responsible for processing move-ins/move-outs
  • Completes initial intakes and assesses housing readiness of potential clients
  • Design Service plans for each tenant/jobseeker on caseload
  • Coordinates interventions and services for participants
  • Manages lease executions, internal accounting processes and entitlement requests from the Department of Social Services
  • Meets regularly with participants to update service plans, while assisting them in obtaining needed services/benefits and entitlements
  • Works with program participants to pursue alternative housing 
  • Schedules and performs home visits on a regular basis, monitoring housing related issues
  • Establishes and maintains positive relations with each landlord
  • Assists the Service Coordinator in managing rent payments and turnover
  • Assists clients with budgeting and saving through financial literacy workshops
  • Links families with community-based, mainstream social service providers that are able to meet their long-term social support and service needs

As it pertains to Employment:

  • Collect and assess information about education, previous employment experience, skills, interests, and personal information
  • Refers clients to employment, job readiness, vocational, and training programs when appropriate.
  • May include aptitude and skills assessments to better identify clients understanding of their vocational interests
  • Conduct mock interviewing
  • Assist in composing a functional resume via workshops and one on one assistance
  • Promotes assisted and independent job search using the internet, job development unit in addition to networking with community based organizations
  • Coordinates with Employment Specialist to identify employment opportunities and job fairs 
  • Tracks services provided to clients and completes required program reports timely
  • Attends all relevant community/service provider meetings 
  • Communicates with all collateral agencies to ensure participant compliance with Employability plan
  • Documents employment service activities in database
  • Collects and inputs service data and participates in program evaluation and quality assurance activities

REQUIREMENTS:

  • Bachelor’s Degree in Social work/Psychology/Marketing or related field.
  • Familiarity with the chronically Homeless population and strong ability to render employment services
  • Must be able to travel at minimal 3-4 days a week
  • Should possess strong interpersonal skills and the ability to build a rapport with  client and service providers
  • Must maintain the highest level of confidentiality and protect sensitive data at all times (HIPPA)
