Case Manager Supervisor - NYC Shelter $50-60K

JOB TITLE:              CASE MANAGER SUPERVISOR

REPORTS TO:        DIRECTOR OF SOCIAL SERVICES

LOCATION:             MANHATTAN, NY

SUMMARY: The Case Manager Supervisor is responsible for supervising the case managers of a 140 bed men’s homeless shelter. This role ensures that all clients in the shelter are assessed and referred for the appropriate services, while also providing case management to clients.

DUTIES AND RESPONSIBILITIES: 

  • Supervises and participates in case management services provided to clients.
  • Participates and facilitates interdisciplinary team meetings to discuss and overcome client barriers
  • Conducts case conferences with clients in regards to non-compliance in order to promote a change in behavior
  • Responsible for the development of service plans for all residents with the Director of Social Services.
  • Meets with clients to assist with development of treatment, vocational training, continuing education, and in-house meeting schedules and document compliance.  Provides intervention for the clients related to issue within the facility.
  • Responsible for conducting intake interviews and processing move-ins of new clients, monitoring and maintaining full occupancy.
  • Responsible for developing service plans with clients. 
  • Responsible for issuing all warning letters and other disciplinary reports and assuring the distribution to the appropriate parties (DSS, out-patient treatment programs, NYS Parole, Probation or Drug Treatment Court) with the approval and assistance of Director of Social Services and Program Director.
  • Responsible for preparing reports and analysis as required.
  • Other duties as required.

EXPERIENCE, EDUCATION AND SPECIAL REQUIREMENTS: 

Requires a Masters degree and prior social service experience preferably working with substance abuse, mental health and homeless populations.  A Bachelor’s degree in Social Work, Psychology, or related field with supervisory experience or a minimum of five years of human services can be substituted. Bi-lingual (Spanish) a plus.  Position requires a valid drivers license and own vehicle. Require some evening and weekend hours. Must have excellent written/verbal communication skills.
