Entitlement Specialist - Bronx, NY

JOB TITLE:                            ENTITLEMENT SPECIALIST

LOCATION:                           BRONX, NY

SUMMARY:

The Entitlement Specialist helps clients address their benefits needs.  Assist with navigating the assorted agencies to address barriers within a community setting that is consistent with their goals. 

DUTIES & RESPONSIBILITIES:

  • Assessment and identification of residents needs
  • Coordinate, monitor and evaluate benefit services
  • Develop and maintain a directory of information for referral services
  • Advocate for residents with other city/state agencies
  • Escort residents to appointments throughout NYC.
  • Assist in medication calls as required and in compliance with all rules and regulations.
  • Assist with intakes
  • Assist Case Managers with maintaining updated case files with necessary documentation
  • Assist Case Managers in keeping records of meeting and actions taken by maintaining physical files as well as electronic files within CARES.
  • Assist Case Managers in interacting and evaluating new and existing client's needs to determine their immediate needs, functional capabilities and assistance needed to stabilize housing, healthcare, finances, etc.
  • Attend and participate in all required agency meetings: staff meetings, house meetings, in-service training and weekly case review.
  • Escort clients to appointments such as Public Assistance, Social Security, and Medical etc.
  • Logical Problem Solving: identifying problems and utilizing a systematic approach to address the problem

EXPERIENCE, EDUCATION AND SPECIAL REQUIREMENTS:

High School diploma required; Bachelor’s degree in relevant field preferred. Two years of relevant experience with special need population facing strong barriers towards benefits. Bilingual Spanish preferred. Excellent communication and computer skills. Comfortable working closely with others in a stressful environment.
