JOB DESCRIPTION

Job Title:            Service Coordinator - Vernon Plaza Family Center

FLSA:                    Exempt

Department:  Homeless Services

Location:         Mount Vernon ,NY

Reports to:     Director, Homeless Services

SALARY:               $60,000 - $70,000

 

Summary

Responsible for the daily operations and delivery of services to families in a Tier II emergency family shelter.  Works as the on-site lead to coordinate daily operations between the Case Management, Childcare, Facilities, Security, and Youth Services departments as well as on-site providers. Proof of COVID-19 vaccination required.

 

Duties and Responsibilities

  • Plans and organizes case management services to meet NYS OTDA and Westchester DSS regulations.
  • Develops and carries out monitoring systems to guarantee quality and efficiency of case management services.
  • Coordinates with the Facilities department to ensure the physical plant operates safely, comfortably, and in compliance with state and county regulations and policies.
  • Responsible for all quality assurance and statistical reporting using ClientTrack HMIS and other database systems.
  • Liaises with Westchester DSS, schools, and other service providers to ensure continuum of care for families as well as to guarantee follow-up in a timely fashion.
  • Provides clinical expertise to case managers in identifying all family members’ needs and developing comprehensive plan to address these needs with clear time frames.
  • Conducts supervision, training, and regular formal performance reviews for direct reports.
  • Represents Westhab publicly with other service providers, government agencies and local county groups.
  • Submits accurate, timely reports as requested.

Education and Experience Requirements

Master’s Degree in social work or related field preferred and four years of experience in supervision and management capacity. Must have excellent case management, assessment, computer, verbal and written communication skills.  Possess the ability to work independently, assess priorities, take initiative and handle multiple assignment. Excellent interpersonal skills are essential.
